POSITION PURPOSE:
In the Acute Care Department, the Social Worker is responsible for facilitation of nursing home placement, discharge planning, crisis intervention, substance abuse treatment referrals, coordination of swing bed (SWB) activity programs, and hospitalized terminal care patients. The Social Worker assists patients with their adjustment to hospitalization, chronic/acute illness, and/or terminal diagnosis. During emergency or sudden death situations, the social worker is contacted to help grieving family members obtain services and complete necessary paperwork. The Social Worker is a grief specialist, financial counselor, discharge planner, and patient advocate who assist patients in navigating community resources and referral information. S/he will support and role model SPH’s values and behaviors

KNOWLEDGE AND EXPERIENCE:
The position of Social Worker requires that the person possess the following qualifications:

  1. Master of Social Work from an accredited program with two years clinical social work experience in a hospital or nursing home setting preferred, or:  Bachelor of Social Work from an accredited program with four years clinical social work experience in a hospital or nursing home setting.
  2. Current and effective State of Alaska social work license.
  3. Experience in comprehensive, multi-disciplinary care planning and assessment.
  4. Excellent communication skills.
  5. Ability to work independently and as a collaborative team member.
  6. Knowledge of social work skills, and understanding of the principles of patient self-determination.
  7. Knowledge of patients rights, as outlined by the OBRA law.
  8. Knowledge of local community and agencies and State-wide treatment facilities, preferred.
  9. Knowledge of Federal and State Regulations, relating to Medicare/Medicaid population and jurisdiction of service providers.
  10. Knowledge of group dynamics and skills in leading a group required.
  11. Knowledge of geriatrics, death and dying, and cultural diversities preferred.

ENVIRONMENT:
South Peninsula Hospital is a combined 22-bed acute care hospital and 28-bed long term care facility located on the southern end of the Kenai Peninsula. The position requires a moderate amount of walking, standing and sitting. The person in this position may be exposed to infectious disease, including all types of blood-borne pathogens, muscle strains, and falls. Generally physical demands are light and work is performed inside the facility. Work hours are set; however, there may be an occasional need for services after hours, when overtime rules would apply.
