HOME HEALTH RN/CASE MANAGER (PCN804)

Utilizes the patient centered care model to plan and provide total, direct patient care in the home, according to established standards, policies and best practices.
KNOWLEDGE AND SKILLS:
The position of HH RN/Case Manager requires that the person possess a current license to practice as a registered nurse in the state of Alaska; One year of recent clinical nursing experience; a valid Alaska driver’s license and vehicle liability insurance in accordance with Alaska state law. Sufficient oral and written skills are required to effectively communicate and document care given. Numerical ability is required to calculate drug dosages. Must possess, and maintain, a current Basic Life Support Certification. On Call after hours and weekends will rotate between RNs when needed for end of life patients.
