POSITION PURPOSE:
Overall coordination of nursing activities within the Long Term Care (LTC) Facility directed toward providing a healthy, supportive environment for the delivery of quality resident care. Functions as a resource person and supervisor for staff working in LTC. Assists with the planning, implementation, and evaluation of policies and practice standards. Serves as a liaison with other hospital departments, as well as community agencies where appropriate. The Clinical Care Coordinator(s) may function in the role of floor nurse/charge nurse by administering medication and treatments to the resident, oversee, monitor, and coordinate care performed by assigned staff, and/or may function as RN on duty per State/Federal regulations.

KNOWLEDGE/ EXPERIENCE: 
  1. Must possess a current license to practice as a Registered Nurse in the State of Alaska.
  2. Must keep current with CEU credits as required by the State of Alaska.
  3. Maintains BLS certification. 
  4. A minimum of three (3) years’ experience in Gerontology, preferably in a Lon g Term Care facility, is required.
  5. A minimum of two (2) years’ experience in a Medical/Surgical setting is required.
  6. Speaking, listening, and writing skills required to effectively communicate with others, and to document care provided.
  7. Computer skills for documentation.
  8. Numerical ability is required to calculate drug dosages.
  9. Must complete general and licensed staff orientation for LTC.
  10. Need to be well organized, detailed orientated, produce quality, accurate work on multiple projects in a fast-paced environment.
  11. Supervisory Skills/aptitude.

ENVIRONMENT:
South Peninsula Hospital is a combination 22-bed critical access hospital, and 28-bed skilled LTC facility located on the southern end of the Kenai Peninsula. The position requires walking and standing for prolonged periods. Stooping, sitting, and the ability to lift large loads without injury to self or others persons. Persons in the position may be exposed to blood-borne pathogens and infectious disease, as well as falls, needle stick injuries, and muscle strains. Mechanical lifts are available to lifting residents as needed. Generally, the physical demands are moderate, and most work is performed inside the building. Working hours may fluctuate based on the demands of the work and scheduling needs.
