POSITION PURPOSE:
Delivery of age-specific, direct resident care. The South Peninsula Hospital (SPH) Long Term Care (LTC) Nurse Practitioner (NP) provides nursing care to residents as the nursing home’s primary care provider, utilizing the Nursing Process and within the framework of established standards for nursing practice.
NATURE AND SCOPE:
The NP is accountable for the delivery of quality care to residents and families, according to the individualized needs and preferences of the resident. The NP is responsible for the assessment and treatment of residents of all ages including adult and geriatric, as applicable. The NP must be competent to provide holistic, resident-centered care with an understanding of the cognitive, physical, emotional/psychosocial, spiritual, and aging process. The NP accepts and promotes professional development as an integral part of nursing practice. The position coordinates resident care activities with other members of the LTC team in a collegial relationship. The NP performs all duties in a manner that supports the team concept, collaborative caring and reflects the philosophy of the LTC department and the mission of SPH. The NP participates in interdisciplinary team discussions regarding resident care issues, medications and treatments, resident and family education.
KNOWLEDGE AND EXPERIENCE:
- Education: Graduate of an accredited / approved School of Nursing.
- Licensure / Certification: Licensed as an Advanced Practice Registered Nurse in the State of Alaska.
- Current Basic Life Support Certification (BLS).
- English Language: Must be able to read, write and speak English clearly
- Will be required to do on-the-job training both in the LTC and at off site clinics per LTC Medical Director’s discretion.
- Other qualifications: Schedule varies to meet department needs. Ability/willingness to be on call, as required.
- Must be able to prioritize multiple tasks, and work with a variety of health care team members effectively.
- This individual must have a working knowledge and experience with computer applications.
ENVIRONMENT:
SPH LTC is a 28 bed long term care facility, co-located with a 22 bed critical access hospital. The environment is generally pleasant, clean and well maintained. The position requires walking, standing for extended periods, stooping and the ability to lift 50 pounds. Persons in this position are exposed to infectious disease including blood-borne pathogens; muscle strains, falls, and needle stick injuries. Generally, physical demands are moderate and most work is performed inside. Working hours may fluctuate based on scheduling needs and resident census.
