Member of the Long Term Care  (LTC) Facility interdisciplinary health team who oversees the LTC Social Service (SS) and Activity Programs.  This individual assures that the residents have a primary advocate and assists each resident to retain or improve his/her everyday physical, mental and psychosocial needs and maintain the highest quality of life possible.  In addition this person works to assure that each resident is provided activities of preference that are appropriate to individual and group needs and interests of each resident, promoting opportunities for engagement in normal pursuits.  The individual in this position works under the direction of the Director of LTC, and working with the nursing staff, strives to assist residents to maintain and/or regain their optimal state of activity, or to provide psychosocial support and comfort to the resident and their family. 

  1. Must be at least twenty-one (21) years of age.
  2. Bachelor of Social Work or related degree from an accredited program is required.  Masters in Social work or related field and/or geriatric certifications a plus.    
  3. Experience working with the Geriatric population with two years (2) clinical Social Service experience in a Nursing Home Facility, hospital or similar setting is required.
  4. Basic knowledge of Medicare, Medicaid, Medicare D and other payer sources required.
  5. Knowledge and understanding of Resident Rights, Abuse Prevention and Resident Centered Care required.
  6. Knowledge of local and State agencies and services provided, is preferred.
  7. Basic understanding of Federal and State regulations pertaining to LTC is preferred upon hire, required by the end of first employment year.
  8. Must have the ability to work independently and as a collaborative team member.
  9. Use of Microsoft Word, e-mail and ability to complete computer documentation is required.
  10. Technical writing skills and/or knowledge of Policy/Procedure development preferred.
  11. Excellent speaking, listening and writing skills required for effectively communicating with others, to document care provided, to formulate letters to residents/legal representatives and to assist with draft policy development is required.
  12. Current Alaska driver’s license with ability to drive the LTC van.  Able to be accepted on SPH driving insurance.
  13. Maintains current Basic Life Support certification. 
  14. Must complete general and Social Service orientations for Long Term Care.

The individual in this position oversees, affords guidance to and will provide annual evaluation input for the staff working in the SS and Activity Programs. Providing input for the staff evaluation process for other LTC staff in relation to resident rights, resident centered care and SS training issues will be expected. The person in this position may be called upon to assist in working with trainee RN/LPN/CNA students during classes and will assist with orientation of newly hired LTC staff members. The LTC Social Services/Activity BSW is responsible for coordination and oversight of the LTC volunteer staff. 

South Peninsula Hospital is a combination 22-bed critical assess hospital and 28-bed skilled LTC Facility located on the southern end of the Kenai Peninsula. The position requires walking and standing for prolonged periods, stooping, sitting, and the ability to lift large loads without injury to self or others.  Persons in this position may be exposed to blood-borne pathogens and infectious diseases, falls, needle stick injuries, and muscle strains.  Mechanical lifts are available for lifting residents as needed. Generally, physical demands are moderate and most work is performed inside.  Working hours may fluctuate based on the demands of the work and scheduling needs.
